LIV Health brings every element of counseling and case management directly to the patient, right in the patient’s own home, nursing facility, or assisted living facility. Our caring, compassionate staff operates under a “patient first” mindset, working one-on-one to develop, then build upon, a solid relationship of trust, security and peace of mind.
We understand how intimidating it can be to look at a towering stack of paperwork and wonder, “Where do I even start?” We’ve been navigating the system long enough to know exactly where to start and how to use our knowledge and resources to arrive at the best outcome for you! We can help with: Medicaid, Medicare, Disability, Social Security, VA Benefits, Aid and Attendance, Enrollment, DIC & Spousal Benefits, Connecting with Resources, Behavioral Health Services and so much more.
No office visits. No long and tedious intake process. Just you and your personal counselor, sitting down together in the comfort and convenience of your home, nursing facility, or assisted living facility. We’ll work one-on-one to establish a plan for continued counseling that best fits your specific needs and schedule.
